Jack McIntosh LaFollette

January 5, 1961 — May 11, 2018

Jack McIntosh LaFollette, 57, passed away after a valiant battle with brain cancer. He was born on January 5, 1961 to Everett and Margaret LaFollette in Murray , Utah . He served an LDS mission in Canada. He married Joann Mcwilliams. They had a daughter, Victoria. They later divorced.
He married the love of his life, Shauna Smith on Sept. 11, 1987.They were later sealed in the Logan Temple. He served in the 854th Army Reserve Unit in Logan, UT. He graduated with a Bachelor degree in Business Management from USU.
He was lifelong member of the LDS church and served faithfully in many callings. Jack had a strong testimony of the Savior and the Gospel. He was a hard working, kind man. He owned and ran Willow Valley Construction. He is survived by his wife, Shauna LaFollette, sons, Trevor LaFollette and Tyson LaFollette; daughters,Victoria (Paul)Kingham and Ariel( Jared) Goodwin. His parents, mother-in-law, June Smith, brothers, Ray(Sue)LaFollette and Paul(Kathey)LaFollette, brother-in-law Chris(Allison) Smith ;sisters,Eve(Doug) Orr, Melinda Ambrose and Amy(Tom)Pondolphino. One grandson(Owen). He was preceded in death by, brother Mark LaFollette, brother- in- law Shawn Ambrose ; Father-in- law, Cleve Smith and nephew Steven LaFollette.
A viewing will be held on Friday,May 18, from 6-8p.m. at the Nelson Funeral home at 162 East 400 North in Logan. Funeral service will be at noon on Saturday, May 19,2018 in the Hyrum North Stake Center,245 Apple Drive in Hyrum with a viewing from 10:00 to 11:30 a.m. Interment will follow at the Hyrum City Cemetery.
